Study finds NHS checks miss up to 95% of younger high-risk women

Study finds up to 95% of women under 50 who are considered at higher risk of developing breast cancer are not being identified by current checks conducted through the UK health system, researchers warn. The study, based on an analysis of screening pathways and risk-assessment criteria, highlights a substantial gap between existing screening processes and the population of younger women with elevated risk factors.

Researchers behind the work say the shortfall stems largely from the design of prevailing programmes, which focus screening invitations and routine mammography on older age groups. The gap means many younger women with a strong family history or other clinical indicators may not be flagged for enhanced surveillance or preventive referral through the usual pathways established by the NHS.

The authors recommend a review of how risk is assessed and acted upon in primary and secondary care, noting that timely identification enables preventive measures and earlier diagnostic assessment. While the study does not prescribe a single technical solution, its conclusions call for examining referral thresholds, improving collection of family-history information in routine appointments and ensuring clearer links between primary care risk assessment and specialist services.

The findings add to ongoing discussions about population screening strategy and targeted risk-based approaches. The researchers state that further evaluation is needed to determine how changes to current checks might be implemented without disrupting existing services and to assess the potential benefits and resource implications. Policymakers and clinical leaders will be asked to consider the evidence as part of broader efforts to reduce avoidable delays in diagnosis for younger women at elevated risk.
